What is a patient health portal and why should I get one? 

  • The patient health portal is your personal electronic chart that syncs with our electronic medical record system. The health portal allows you to directly communicate with your providers, check your progress notes from each appointment, view lab results, view and make appointments, and more. 


How do I get access to my patient portal?
  • For existing patients, call the office to "web-enable" you. This will send your login information to the email provided in your chart. Otherwise, ask the front desk for your login information at your next appointment!

  • For new patients, when you call to make your first appointment and provide us with your e-mail address, you should receive a same day e-mail with your login information. It is not necessary to "pre-register," but make sure to fill out the Medical History "Questionnaire" as part of your required new patient paperwork. See new patient tab.
